We are seeking a compassionate and motivated Nurse Practitioner to join our virtual care team. This fully remote position offers the opportunity to provide accessible, personalized, and continuous primary care to patients across Illinois — from the comfort of your home.

Key Responsibilities
  • Deliver comprehensive virtual primary care services via video and phone consultations, including routine health maintenance, chronic disease management, preventive care, and acute issue management.
  • Conduct remote assessments, order and interpret diagnostic tests, and prescribe medications as clinically appropriate under applicable telehealth and state regulations.
  • Develop and manage individualized care plans in collaboration with patients, families, and interdisciplinary care teams.
  • Coordinate referrals, follow-ups, and continuity of care with specialists, pharmacies, and community resources.
  • Provide education and counseling to patients on preventive care, treatment adherence, and self-management strategies for chronic conditions.
  • Maintain accurate and timely documentation of all patient encounters in the electronic health record (EHR).
  • Ensure compliance with all telehealth clinical standards, privacy laws (HIPAA), and organizational protocols.
Qualifications and Job Requirements
  • Active Nurse Practitioner License in Michigan (required; multi-state licensure a plus).
  • Master’s Degree in Nursing or higher (required).
  • 2+ years of experience in primary care, family medicine, internal medicine, or urgent care preferred.
  • Demonstrated ability to provide safe, effective, and empathetic care in a virtual environment.
  • Strong clinical assessment, diagnostic, and communication skills suited for remote patient interactions.
  • Comfortable with EHR platforms, telehealth technologies, and digital communication tools.
  • Organized and self-motivated, with excellent time management skills and a commitment to patient-centered care.
  • Prior telehealth experience is highly desirable.
